Required Documents and Fees

Timber cutting licenses (Jack trees / Palm trees / Female Palmyra)

  • A copy of the deed certified by the Grama Niladari and verified by the original
  • Where the noticeable owner of the land is certified by the Grama Niladari when the applicant is not aware of the clear title of the land
  • Affidavit taken to prove that there is no objection to the issuance of a permit to rent to the applicant when there are several heirs
  • If a new construction is expected to throw down trees, an appropriate construction plan
  • Letters of the pradeshiya Sabhas / Urban Councils if they are being scattered due to dangers
  • If court verdicts regarding the land are relevant, court orders regarding them and dump
  • Notice of correspondence in the case of power line disturbance or protest
  • If the tree is located on the fence, the approximate (bare) land is owned by the owner. The owner is not opposed to cut the tree

Issuing permits for timber / timber transport.

  • Completed application
  • Grama Niladhari report on timber, timber or timber
  • License for cutting Cutting Jack / Palmyra / Female Palmyra Tree
  • Timber logging document certified by Grama Niladari
  • Site Recommendation
  • Original copy of the transport license and the official receipt issued by the Department of Railways (Adavi Vana reports, Grama Niladari Reports or not necessary and License Duty does not apply) Licenses Obtaining licenses to re-transfer Railway /
  • Invoices for timber, cracked timber or timber goods

Extension of Timber / Extradition Period

  • The initial police complaint
  • Original copy of the license
  • Gratitude report on non-transportation

Issuing license to open new pawning centers

  • Grama Niladhari report
  • Superintendent of Police
  • Original registration certificate and another copy
  • Affidavit not to have a lawful and irregular act
  • Original copy of insurance certificate covering bribery and burglary allegation and another copy
  • A joint venture partnership
  • A copy of the tax deed

License fee - Rs. 1000.00

Deposit - Rs. 100000.00

Registration Charge - Rs.25000.00

Renewal of the pawning center license

  • Last year's License and a photocopy
  • A valid insurance certificate and a photocopy for the year
  • The financial report of the year and a copy of it
  • Affidavit

License fee - Rs. 1000.00

Vehicle Revenue License

  • C.M.T. Certificate of Registration or Registration of Motor Vehicles. Or 76 M.T.A.11 form
  • Insurance Certificate
  • Last certificate
  • Issuing test report

Issuing liquor license

Approval letter issued by the Commissioner of Excise

(Charges - according to the Gazette notification).

Renewing of liquor license

  • Approval letter issued by the Excise Commissioner
  • Clearance Certificate issued by the Assistant Excise Commissioner General
  • Clearance Certificate and its copy issued by the Inland Revenue Commissioner and the Provincial Revenue Commissioner
  • Last year's license

Permission to hold liquor whilst at ceremonies

  • Invitation card

Issuing toddy permits

  • Completed Tender Application
  • Original Valuation Certificate
  • Original receipt of receiving the tender form

Issuing Animal Transportation Licenses

  • Application - Schedule 02
  • Recommendation of Grama Niladhari
  • Veterinary certificate
  • Recommendation of the vehicle used for the transport of animals
  • Receipt for payment of Rs. 50.00 per animal to the North Western Provincial Council

Recommendation for Gun Permits

Application completed by the Grama Niladari and the relevant police station by the applicant

Excavation of sand gravel

  • Formally filled application
  • Copy of deed.
  • Copy of Plan (Not compulsory).
  • If the applicant is not the landlord, an affidavit or tax declaration obtained from the landowner should be submitted

Transport of sand, gravel and soil

  • Formally filled application.
  • A copy of the mining license of the miner.
  • Copy of the Deed of the excavation land and the land obtained
